Information we collect

When you request an appointment or contact us, you may choose to provide your name, phone number, email address, city or appointment location, document type, preferred time, and appointment notes. Please do not send Social Security numbers, government identification numbers, financial account numbers, medical records, or document images through the website contact form.

How information is used

We use the information you provide to respond to your request, confirm availability, prepare an appointment quote, communicate about a scheduled appointment, and maintain ordinary business records. We do not sell or share personal information for cross-context behavioral advertising.

Website-form delivery

Appointment requests are transmitted through our website host and FormSubmit, a form-delivery provider, to our private business inbox. FormSubmit states that submissions may be retained in its archive for up to 30 days. Email providers may also process the request as needed to deliver and store it. Do not include identification images, document images, financial information, medical records, or other sensitive records in the form.

Cookies and analytics

With your permission, we use Google Analytics to understand website visits, Call and Text button clicks, and successful appointment-request submissions. Analytics uses cookies and processes device, browser, approximate location, and interaction information. Call and Text clicks do not tell us whether a conversation took place.

We do not send your form entries, name, email, phone number, appointment notes, or document details to Google Analytics. Website query strings and fragments are excluded from the page addresses we send, and referrers are limited to their website origin. Advertising personalization and Google signals are disabled.

Analytics stays off until you choose to allow it. You may decline or change your choice below; the contact form, calls, and texts work either way. We also keep analytics off when your browser sends Global Privacy Control or Do Not Track. Your choice is stored on this browser.
